One of the issues that keeps coming up in workshops on end of life issues and ritual creation, is the need for some ritual or prayer that can be said at the time we sign our advance directives and health care proxy forms. These are very real acknowledgements of our own  mortality and are often signed in the lawyers office without ceremony or recognition that is a powerful spiritual moment.
